Required Documentation: For each medical travel assistance application, shareholders must submit documentation confirming travel (such as flight itineraries or boarding passes) and medical appointment verification. Applications cannot be processed without both items.

Required Documentation: For each funeral assistance applications, applicants must submit documentation supporting eligible funeral expenses (such as itemized invoices or receipts) and any additional information requested by the Foundation. Applications cannot be processed without required documentation.

Assistance is limited to eligible expenses, up to $5,000, and must be submitted within six months of the date of death.
